Don’t ask for more data than you really need. Make it as easy as possible for people to fill out the form.
You also want to keep the form length to a minimum to save on printing and paper costs. If you expect to print hundreds, even thousands, of copies, the costs can add up quickly. In most cases, your registration form does not need to be longer than one page.

For this tutorial I will create a registration form template. I will use this template for registration form in Word. This template is one of the print templates available in Envato Elements.

Add the registration template to an existing project in your Envato Elements account. You can also create a new project. Click Add and Download.
You can find the registration template in your Envato account at any time. It will be available under the My Downloads area. You can also download it there in the future.
Modeling Form: Fill Out & Sign Online
To extract the file on a Mac, find it in Finder, then double-click the file. Or right-click the file and then click Open.
You can open and read the Info Documentation file in the Info Documentation folder. It also has the link to the Roboto font used in the template if you don’t already have it on your computer.
The Info Documentation file has additional instructions and the designer’s email address. The email address is handy so you know how to reach out if you get stuck using the template.
Model Application Form
Expand the Microsoft Word folder and the subfolder that has the desired print registration form size. You can choose between US Letter size for 8.5 x 11 inches and A4 for 8.27 x 11.69 inches.
TIP: Duplicate either “Registration Form.doc” or “Registration Form.docx”. Then adjust the copy. Do not touch the original file. That way, if you mess up the file, you can duplicate the template again and start over.
In this case, I don’t need the large text next to the logo and the rectangular box in the upper right corner. The box serves as a placeholder for the registrant’s photo. We can delete those elements.

Add your address, phone number and email below the logo. Double click in the text box to edit the text. Type your information in the text box.
At this point, you can change the font, font size, color, style, and alignment of the form title. To do this, select the text box. Select the settings you want under the Home tab.
Remove the fields you don’t need. For example, to remove the place of birth, select the text box and then click the Delete key. Or right click on the text box and click Cut.

You will see, this leaves white space in the middle of the line. To remove the white space, delete the gray rectangle at the end of the line.
To remove multiple elements at once: Click one, hold down the shift key, then click other elements. Press the delete key.
For this registration form template, I will delete the rest of the fields under Personal Information.

If you need to add form fields, the easiest way is to copy an existing field. In this case, I’ll copy the Membership Type field and use it to create a Payment Details field.
When the registration form looks exactly the way you want it, it’s time to print the form.
TIP: Print extra copies because you may need more than you think. It doesn’t hurt to be prepared!
